DEAR SCHOLAR-ATHLETES:

I hope you had an enjoyable summer and ready to embark upon another challenging year of collegiate athletics for the Spelman Jaguars! The beginning of a new academic year is fast approaching and it is necessary that you take time, with your parents to review and complete all forms prior to the first day of your participation.

The NCAA and the Institution require all scholar-athletes to be cleared by the Team Physician and all forms completed prior to your participation, therefore, no forms, no participation. We are requesting all scholar-athletes adhere to the scheduled examination date and time for your respective sport. If you have a conflict with the scheduled date and/or time, you are required to contact your coach and the Sports Medicine Department as soon as possible so further arrangements may be made.

All pre-participation physical examinations will be conducted in Read Hall room 103. Scholar-athletes will be assigned an appointment time only when we have received all forms and a cashiers check or money order in the amount of $65.00. Completed paperwork must be received in the Department of Sports Medicine 24 hours prior to the intended date of the physical examination.

FRESHWOMAN & TRANSFERS:
If you are a Freshwoman or transfer, you must provide a copy of the completed physical required for Spelman College Health Services to the Department of Sports Medicine along with all forms to be allowed to participate. All forms and $65.00 payment must be in my office 24 hours prior to the date of your physicals. Upon clearance by the Spelman College Team Physician you will be allowed to participate.

INSURANCE INFORMATION:
The scholar-athlete is covered under the accident medical coverage provided by the Department of Athletics, however, coverage is subject to specific policy terms and conditions and includes certain restrictions and exclusions of which you should be aware. For more information about this insurance coverage, please contact the Department, please contact the Department of Sport Medicine for a detailed benefit summary. Please note that Spelman College assumes no responsibility whatsoever for any uninsured expenses, and we strongly recommend that the student have coverage through a primary health insurer to avoid possible, significant out-of-pocket expenses in the event of an injury.

Please also note, the NCAA’s Catastrophic Injury Insurance Program covers student-athletes who are catastrophically injured while participating in a covered intercollegiate athletic activity (subject to all policy terms and conditions). The policy has a $75,000 deductible and is supplemental coverage in the event of a catastrophic injury. More information on this program can be found on the NCAA's web-site.

Additionally, it is important to note that the insurance provided under the Spelman Student Health Insurance Policy does not cover athletic injuries incurred while engaged in NCAA sponsored practices and events. The Department of Athletics requires any scholar-athlete covered, under other policies be required to provide a copy (front and back) of that policy which will be filed in the Department of Sports Medicine.

Again, should the scholar-athlete be covered under additional/other insurance policies the following must be submitted to the Department of Sports Medicine:

  • Carriers full name
  • If an out of town HMO a list of local doctors covered by that HMO
